Food Vendor, Please Note

1) You must submit a temporary food permit application with the Braintree Health Department
2) the permit will be issued on the day of the event
3) permit fee is $50, payable to the Town of Braintree
4) tent will be provided, however you will need to supply your own table and chairs. Cooking under the tent is not allowed.

We will walk you through the steps to obtain the food permit as soon as we receive this completed form.
